Student Accident Claim Form

ABC (Accessing Benefits for Children) – Program to assist you in getting health insurance for your child.

Healthy Families - Medi-Cal for Children - (888) 747-1222 (toll free).
Provides low-cost health, dental and vision insurance for children 1 year old up to age 19. Covers children without health insurance and those with share-of-cost Medi-Cal. Offers low monthly premiums from as low as $4 per child to a maximum of $27 per family. Does NOT count family resources (such as bank accounts or cars). Allows applicants to mail application.

Champ – Chiledren's Health Access and Medical Program. Provides information on free and low-cost healthcare programs. Also provides program prescreening and enrollment assistance. Call 1-866-742-2273.
